Supporting patients to remain independent in their medicine management through the development of medication tool kits.
Funds were awarded to enable the purchase of tool kits for 4 sites initially. The value of the kits will then be evaluated before rolling out across Fife.
The medication tool kits will contain a variety of devices, for example, inhaler aids, pill splitters and electronic reminders, and key documents such as medication reminder charts. The project aims to support patients and carers retain independence and control around the management of their own medication.
Key staff members at each site will receive training on the ‘medication toolkit’ and how best to use it to support patients and their families.
