Mr Gray wanted to hear more about Fife’s integrated approach to discharge planning, which ensures Fifers can be discharged from hospital in a timely manner after a stay in hospital.
Working together through the Fife Health & Social Care Partnership, colleagues across NHS Fife, Fife Council, and the third and independent sectors have developed a Home First Strategy.
NHS Fife Chief Executive Carol Potter, NHS Fife Board Chair Pat Kilpatrick, Interim Director of Fife Health and Social Care Partnership Fiona McKay and the Partnership’s Head of Community Care Services Lynne Garvey, met Mr Gray and introduced him to one the multidisciplinary teams working in this area.
Staff talked to Mr Gray about their work and emphasised the importance of collaboration across all aspects of health and social care services to support discharge planning, and the difference this person-centred approach makes to ensure patients are not unnecessarily delayed from returning home or to a homely setting.
