High Health Gain
High Health Gain (HGG) are a group of patients, mainly older. They all have more than two health conditions and have been in hospital several times. In Fife we are using a case management model, with the District Nurses (DN)’s to support these patients.
A full assessment, with their consent, is done. We look at medical, nursing, social and family factors. All this is done in partnership.
We know that putting the right things in place can reduce hospital admissions and improve the quality of life for these patients. This can include reviewing medications, looking at care needs, etc.
