The Transition Nurse offers health needs assessments and support to young people with learning disabilities, between 14-19 years of age, throughout Fife, to make the transition from adolescence to adulthood.


The nurse will endeavour to ensure high quality uninterrupted healthcare occurs between child and adult services. The aim is to help the young person achieve their potential in terms of health, development and well-being.


The Transition Nurse has a key role in liaising with other health disciplines and agencies to assist with pro-active and co-ordinated care plans, in addition to future planning for services.
