Community Specialist Palliative Care Nursing Service
The Fife Community Specialist Palliative Care Nursing Team takes the Hospice’s knowledge and expertise into the community. The team works closely with home-based patients, their families, carers, GPs, District Nurses, Hospital Consultants, Specialist Hospital Nurses and other health professionals in developing programmes of care best suited to patients’ individual needs and circumstances. They also provide training to Community Nurses and staff in local nursing homes.
Our Community Specialist Palliative Care Nurses are part of a wider team of Consultants, Specialist Registrars, Doctors, Nurses, Therapists and Social Workers. In Fife there are seven Community Specialist Palliative Care Nurses, each allocated to a specific area but may overlap when service needs demand this. Each patient will have their own named Nurse, who will work with the wider team to support the patient, their GP and their District Nurse.
The main aim of the Community Specialist Palliative Care Nursing Team is to support a person and those important to them throughout their illness, to ensure he or she achieves the best possible quality of life, with relief from symptoms, offer emotional support and practical advice to people with advanced illness and their families and a flexible choice about where he or she is looked after.
Referrals to the service may be made by health professionals following discussion and agreement with the person and their family. GPs permission should also be sought. Healthcare professionals can find more information about how to refer on the Referral Guideline page on the Intranet.
There are two ways to refer to the community service; (1) the referral form, available under the "Publications" tab on the left which should be e-mailed to Fife-UHB.PalliativeCare@nhs.net ; (2) via SCI Gateway.
