Social Prescribing

This programme seeks to help individuals identify important influences on their mental health and wellbeing and take action to improve the quality of their lives. 


General practitioners can refer patients to a Health Coach, who supports 3 GP practices based at Lochgelly Medical Centre, Fife.  Depending on the individual’s needs, the Health Coach provides tiered support; they support access to information as well as referral to connect people with and engage with appropriate interventions.  


The Health Coach facilitates improved access to other services by co-ordinating a referral process that leads to non-clinical community based sources of support, as well as liaison with a range of organisations that provide social, recreational or therapeutic support.


Referrals lead to opportunities for learning, volunteering, employment and physical activity as well as supporting those individuals overcoming bereavement, stress and benefit and debt issues.
