We aim to optimise your health, wellbeing and independence through multi-disciplinary medical intervention and rehabilitation. Our primary focus is frailty. This is delivered in a hospital-based clinic setting.
What should I expect when attending an ARC?
You will be referred to the assessment centre via your GP or by hospital medical staff to be assessed within a clinic setting by a team of medical, nursing and Allied Health Professionals (Occupational and Physiotherapy).
Following referral, your case will be triaged and co-ordination of your input discussed and planned via a multi-disciplinary approach. You will be invited to attend one of the centred closest to your home for assessment. They will ask about your current health and home situation to determine if you have any needs that they can help with. This could include:
- A review of medication
- Your ability to walk (mobility)
- Your balance and strength
- How you’re managing at home with your day to day activities
Although each professional will see you separately to carry out an individualised assessment focused on your needs and goals, the team will then meet to discuss their assessments to gain an overall picture of your health which will define what is required.
Options may include:
- Physiotherapy/Occupational Therapy follow up
- Medical or nursing follow up
- Referral to other community or hospital based services
Referral process
Referrals for ARC are made to SPOA (single point of access) via your GP or a member of the hospital medical staff. Please note that you cannot self refer to this service.

GP Fellow, Advanced Nurse Practitioner, Nurse Practitioner, Nurses, Occupational Therapy and Physiotherapy staff.
